Students who have paid all university fees can obtain their transcripts by ordering online, or submitting a written request to the Office of Admissions and Records, and paying the transcript fee.
Complete required personal information, download the immunization certificate, and follow the directions provided. Upload a copy of your immunization forms to your Med+Proctor account. Make sure your forms are complete and legible.

A high school counselor, teacher, principal or other school official may also email high school transcripts and mid-year reports directly to undergradmail@uic.edu. A hard copy of the final official transcript, however, will be required after admission and prior to class enrollment.
